Perteet Inc. Benefits Summary
Medical (eligible first of the month following 30 days of employment)
Medical coverage is provided by Aetna. This plan is a PPO and offers a built-in Healthfund account of $500 per individual/$1,000 per family. The company funds a Health Reimbursement Account (HRA) in the amount of $500 per employee per year. The HRA rolls over year to year to a $1,500 maximum. The company pays the entire cost of the employee premium and 25% of the cost of dependent coverage.
Dental (eligible first of the month following 30 days of employment)
Coverage is provided by Washington Dental Service, and has an incentive provision that starts at 70% coverage up to 100% for preventative and basic services. Major services (crowns, inlays, etc.) are covered at 50%. There is a $2,000.00 per person maximum per benefit period. The company pays 100% of the employee’s premium, and 25% of dependent premiums. There is a $50.00 per person annual deductible, up to $150.00 annual maximum per family. Deductible is waived for diagnostic and preventative services.
Prescription Drug Benefit (eligible first of the month following 30 days of employment)
Prescription drugs from an approved pharmacy are purchased on a co-pay plan of $15.00 for generic drugs, $25.00 for formulary brand name drugs, and $40.00 for non-formulary brand name drugs.
Life Insurance & Accidental (eligible first of the month following 30 days of employment)
Perteet pays 100% of the premiums for Group Life and Accidental Death and Dismemberment Insurance for each employee through Jefferson Pilot Financial. Benefits are equivalent to one times annual earnings.
Long Term Disability (eligible first of the month following 30 days of employment)
The company provides this insurance coverage in the event that you become disabled and are unable to perform your regular job functions for more than 90 days. This insurance provides 60% of your regular rate of pay, up to $8,000 per month, for the duration of your disability, coordinated with other income. Perteet pays 100% of the premium for this coverage.
401(k) Retirement Investment Plan & ESOP (eligible first of the month following 30 days of employment)
The 401(k) plan is available for all full-time employees through the Standard Insurance Company. The company will match 50% of the amount the employee defers each payday, not to exceed 5% of that payroll. This match goes into the company ESOP.
Section 125 Premium Only Plan (eligible first of the month following 30 days of employment)
This benefit allows payroll deducted premiums for medical and dental insurance for dependent coverage, a medical expense savings account for unreimbursed health expenses and other health related premiums, and child care tuition costs to be deducted from your paycheck on a pre-tax basis, thus lowering your taxable income. Maximum health-related deductions, not including payroll deducted premiums, are $3,000 per year per family, and maximum child care/tuition deductions are $5,000 per year per family.
Paid Holidays (eligible immediately)
Regular full-time employees are eligible for 8-1/2 paid holidays each year.
Paid Vacation (eligible following 6 months of employment)
Regular full-time employees accrue vacation at a rate of two weeks per year during their first through fifth year of service. Beginning in the sixth year of service, vacation accrues at a rate of three weeks per year.
Paid Sick Leave (eligible immediately, as accrued)
Regular full-time employees accrue sick leave at a rate of 10 days per year.
Tuition Reimbursement/Professional Societies (eligible following one year of employment)
Perteet encourages employees to increase their knowledge and abilities. To this end, Perteet may reimburse employees up to 100% of the cost of tuition, fees and materials associated with pre-approved, successfully completed courses of study. Perteet also encourages active participation in technical and professional societies and will pay annual dues to one such organization for each professional employee. Additionally, Perteet provides for time off and reimbursement for costs associated with gaining your professional designation (PE, EIT, PLS, LSIT and AICP).
